Reporting a bird strike
To submit a bird strike report to the Department of Civil Aviation, the involved parties shall proceed as follow the report shall be submitted when there is a collision between a bird or a flock of bird and an aircraft.
Pilot submit a report by using form 1 (Bird strike reporting form) as soon as practicable.
After the occurrence and the operator has already assessed the damage value, the operator submit a report by using form 2 (Supplementary bird strike reporting form)
for the operator of temporary takeoff and landing area shall fill the report using form 1 in case the bird strike or wildlife strike has been observed.
The completed form shall be sent immediately to the Department of Civil Aviation by mail, fax or email.
